The list of documents to be submitted at the time is as follows:

• Admission forms are filled only at the time of admission otherwise enquiry forms.
• 4 photographs  of the child and one photograph of Father & Mother .
• One certified true copy of the child birth certificate.
• If it is an adopted case than the related documents of adoptions is required.
